Description of the Cognitive Assessment Appointment:
The "Cognitive Assessment" is a two and half hour appointment. One hour for history, background, record review, one hour for cognitive testing and 30 minutes for feedback and treatment recommendations.
This Cognitive Assessment appointment is $300. Please note that you will not be scheduled with the Psychiatrist, nor will medications be prescribed at the time of the Cognitive Assessment Appointment. Please note that you will be asked for a credit card upon confirming your appointment with a representative. A $100.00 cancellation fee will be charged should you cancel your appointment without 24-hours notice. Your card will not be charged otherwise.
To schedule an appointment with the Psychiatrist for a medication evaluation, you must call 1-866-324-2088.
In order to provide individuals with the highest quality of service, ADD Health & Wellness Centers, Inc. does not contract directly with insurance companies. However, frequently our services are covered by the "out of network" portion of your insurance coverage. Our staff will provide you with the documentation necessary for you to submit your claim to your insurance company.
